Supporting Student Health & Wellness

The Shaw Elementary Health Clinic is dedicated to promoting the health, safety, and well-being of every student. Our school nurse partners with families to provide routine health services, maintain student health records, support students with medical needs, and encourage healthy habits that help students succeed in school.

Use the resources below to find important health information, required forms, and clinic policies.

Medical Forms

You may visit the nurse in the clinic or print forms from the district website.

Immunization Requirements

Ohio law requires students to meet state immunization requirements before attending school. Immunization records should be submitted on or before a student's first day. Students who are not up to date on required immunizations and are not actively completing them may be excluded from school after 14 days, in accordance with state law.
